So what happened to this NGC holder?

Not my coin -- just a completed eBay listing that got a lower price than it perhaps normally otherwise should have.

The description didn't really say much, other than something to the effect of: "please note scratches / damage before bidding."

It could have been just the camera or lighting or something, but the coin also seemed to have some "toning" with a bit of greenish tint to it. Then again, that could be nothing, as other examples of this older coin (75% Ag) from other sellers seem to have that color, as well.

Aside from what appears to be the poor removal of a price sticker..

My theory is that there was at some point, some moisture present, causing the interior label to warp & wrinkle, since these holders aren't completely 100% air-tight. Whether that was just really damp / humid conditions for a long time, or was stored where there was more significant water (e.g., a flooded basement or something), I don't know.

The label on the other side, I should note, looked fine...

    There is a simple explanation.
This style of NGC's label is on the outside of the holder.
